Has anyone had any issues with their supercharger belt coming off the tensioner pulley? Mine keeps coming off a few mms which over time causes it to shred the belt, I thought it was the tensioner pulley going bad so I just replaced it and the belt slid off a few mms as soon as I started it up. Any ideas what could be causing this?

Has your belt been fairly tight each time? Also I would check just to be sure everything lines up

It's been tight, I just don't see what could be causing it to be misaligned even after today when I pulled the supercharger off to replace the pulley.

I wonder if the pulley surface(s) is warped or something

I don't think the belt has even been that tight to cause them to warp, but I guess I can take a look

Re-check the service manual and make sure that the bushings, washers, etc are installed correctly.
