Having a new clutch disk, act pressure plate and flywheel put in this week, this is not giving me a good feeling. Used all OEM bearings also. I hope you get this sorted. Never heard of an OEM bearing failing but I suppose anything is possible.

Likely bearing installed without grease, and the guide sleeve was already warped. No biggie, it's not an expensive part. I've seen this happen before, so has user bserious. The bearing costs more than the sleeve by a factor of 3.

Fixed everything, guy said I need a clutch line though that's where the leak was coming from. One thing though, my clutch pedal feels sticky and is making a squeaky noise, I thought this was something that had to do with not greasing the slave? But I did that when I installed it...

Try putting some grease on the clutch fork tips, that usually helps a bit. Remove the rubber boot and the slave, and you can pull the fork from it's hanger, just slap some honda urea grease on both fork tips where the fork slots into the release bearing. If you have a way to get a better look inside(boroscope works), try to get some on the release guide. Then install everything in, the fork and release bearing will slide on the grease and reduce squeaking.

Shouldn't they have done this when installing the bearing?

They should, but people are people and they sometimes make mistakes, or forget. You know what they say, you want something done right, you have to do it yourself.

So I did some grease but it still squeaked and it's basically sounding like it was coming right from the pedal but I put grease on the pin when I changed the MC... anyways I was just driving home and my whole clutch just went out. There's pressure in the pedal but the car won't go in to gear at all again but unlike last time where I could shift gears it won't go in to any gear at all, wtf is going on

Sounds like the throwout bearing just exploded.

I have never seen a single oem TO bearing fail.

It sounds like they are screwing up the install. I have also never seen a bad clutch hose. They likely cut it with the edge of the transmission.

If the clutch hydraulics are improperly adjusted, the TO bearing will operate at all times and fail quickly.

This is all installers fault IMO.

Another note I've been saying for 14 years, 90% of S2000 clutch MCs leak. 95% of those leaks will stop with simple fluid replacement.

Where are you located?
