Likely all intake retainers are fractured. In a case this bad, very tiny cracks may be found on the exhaust retainers.

I'm usually pretty good about taking my foot off the gas and onto the clutch if i make a mistake but I couldn't tell you how fast I did it at the time

I didn't notice this before, but it looks like the valve was completely split in half... lengthwise. I don't think I've ever seen this before.

I think what you are seeing is the other exhaust valve has broken off its stem and has become embedded in the port of the adjacent valve.
