Such a tragedy.
I have 8 3-ton jack stands, it's not at all unusual for me to use 6 of them when I go under one of the cars.
And never, ever, ever, do I only use 2, allowing the car to be on an angle.
Sad really, the poor guy probably was not even aware of how much of a risk he was taking...

I've seen cars up on two stands on driveways with substantial slope. Scared me to look at it. I've seen enough motorcycle kickstands sink into asphalt on hot days to never trust jack stands on asphalt. And don't even get me started about jack stands on gravel driveways.
I only use mine on the concrete floor of the garage; it's pretty close to level. I generally use 4, which precludes the car rolling but will use 2 with wheel chocks when it's convenient.
I also wonder about the quality of some of the dirt-cheap Chinese jacks and stands I see in the market these days. Low price isn't my first priority when purchasing something that my life depends on. As pathetic as some may consider my life, I want to extend it as long as possible.

If you must use just two jack stands, please use rubber (non slip) wheel chocks on the front/rear tires.
