If you feel you can have a talk with him/her, do. If not, leave a note. If there's anything I've learned this year in particular, it's that you need to speak the truth, and then man-up to the consequences.
That said, you need to identify what you perceive the problem to be. Find examples, listen carefully, and work the problem.
If the problem is he/she isn't willing to face the problem, then part ways knowing you did your best. Leave an opportunity for him/her to make amends.
