its kinda silly, but when people think of people having high ranking in the medical field, they associate that with lots of money, which in alex's case would be fitting

Yea, but even so, my first guess would have been a CEO/President/high ranking official of a well-sized technology company.
But I guess it's because I work in that sector as opposed to the medical field.
