Sunglass experts, check in!
If you care about lens quality, nobody in the world can TOUCH Oakley. Style and other factors do play into things, but their lens production is amazing. I should know, I've seen it first hand. I wear beater glasses daily, but wear a pair of my Oakleys when I'm on a hard drive or care about visual quality.
I F'n HATE Oakleys... their lenses are really good...but the frames, hinges and ear pieces suck ass. I've owned 4 pairs and have finally given up on them.
Maui Jims are the best of what you listed IMO. Their styles are a little big lensed but they have a few that look good.

Oakley also has really good customer service. If your pair is ever broken you send them in, pay $30 bucks and they fix and return them to you. Broken lens, hinge, whatever.
I've owned Ray Bans, Serengeti and Maui Jims. I need glasses with nose pieces because I don't have much of a bridge.
Based on these three brands, I'd say Maui Jims are the best. Excellent lens. Excellent fit. Very light weight. (I have the titanium frame for driving, plastic frame for sports). I am however, comparing my Maui's to glass lens Ray Bans and Serengeti's. A long time ago they use to say, glass lens were better for clarity. I'd say, technology has come along and dis proofed that statement.
I'd recommend trying on a pair of Maui's.

Maui Jims....I bought mine at a golf demo day.
The salesmen told me they were 'air conditioning for the eyes'. I thought 'what a line of BS' until I went outside and compared them to my Oakleys. He had me look at large white tent in the bright sunshine. Oakleys, I was squinting...Maui Jims were pure pleasure.
You'll never go back.

Have you called Revo to price a replacement lens yet? It is another option open to you.
As far as brands go, Maui Jim is pretty much the best you can get - the layered lenses hold up incredibly well and customer service for repairs or replacements is easy. The lenses have protective coatings that make the optics last and last, and they are polarized and free from distortion - glass or plastic. Tghe lenses are good for every day use, or sports with the Sport line. They are all polarized and use a laminated construction technique. My personal favorite sunglass is a Maui Jim Sport Ti - I wear it everywhere.
Serengetti is also anexcellent glass company, but finding their polarized rose glass is hard. Most of its lenses are photochromic so they will lighten and darken with conditions. These are great driving glasses as they change with light condiions and are easy on the eyes. The amber and rose color lenses offer haze free viewing. I don't know whats recent with them, but they are Corning glass and are also excellent.
Oakleys are sports sunglasses and eye protection and are built to such standards. The frames and lenses are light and will stay on when you are active, the down side being the lenses scratch easily - being soft plastic. The lenses are shatter proof and are optically distortion free, so they are great for activities.
Revo's claim to fame was that the glass was the same as that used in astronaut helmets, but I always found the glasses heavy and the designs dated. One thing about Revos though is the lenses are very clear and the frames are strong - qualities of superior construction.
RayBans are you basic quality glass lens. Luxottica owns it and I think Revo as well, as well as a bunch of designer brands. The RayBan green/grey is a good lens, but imo you'll be happier with a Maui Jim, Serengetti, or Oakley glass, depending on your needs.

sorry to the Oakley lover above... Maui Jim optics are BOSS.
but to add value to OP's 3 questions...
1. there is absolutely no way you can judge how a pair looks until you try them on. seeing them in photo, and fitting them on your face is something different
2. scratch resistance = GLASS. however, be aware that glass is more prone to shattering, and is also heavier
the "consensus" on optics quality is that old Revo were very very very nice. nowadays, that title belongs to MJ.
raybans... alright. i personally like the Predator Flight
oakleys... im very ambivalent on this brand. i had 2 pairs of square wires that i did like. however, they have too many outrageous, out-of-this-world design. their plastic frames feel cheap. and they have a very cultish OMG-OAKLEYS-ARE-THE-BEST following that makes them a turnoff.
mj... good optics but most style are very very lame
