Is it required to create holes on your trunk to install this? I just bought an OEM spoiler and it turned out total of 4 holes are needed to install

No. Your 3rd brake light goes inside the spoiler and the studs located in the light go through the spoiler and into the trunk. The spoiler also comes with double sided tape around the edges of it.

Is the spoiler made from full CF or just the bottom side that is made from CF? I was wondering if you can install it w/o having it painted. Just curious.

It is CF on both the top and bottom. It comes with clearcoat, mine was installed without painting.
