Ugh. I've seen one wheel come off at an autocross. Camaro with a bad wheel spacer on the rear. The spacer split, and a chunk fell out leaving a gap between the rim and the hub. The right rear beat the car through the timing lights.

The latest I heard was that the lug nuts that he had just bought had the right thread pitch, but were just a bit too large ID. So they threaded on fine and seemed to torqued down properly. He's a bit annoyed with the shop that sold him the lugs. All things considered, he's probably lucky this happened at the event, as opposed to on the freeway on his way there.
I just read that as well Larry, very true about they were lucky it happened where it did.
I almost had this happen to me with a right/front wheel a couple years ago, I didn't torque it enough and it slowly became loose after a couple laps. I was lucky and caught it before anything bad happened.

I quickly invested in a torque wrench when I started swapping rims for autocross in Phoenix. The brakes would get so hot out there, that even slightly undertightening the wheel would result in loose lugs after a few heat cycles.
