Had a similar issue, except one of our inexperienced technicians at my dealership was to blame. I took a MAP torch, heated the metal, and tapped it back into position. After that, I dropped the car off with a local fabricator who tacked all the tabs on the subframes to help prevent this in the future, and also can be easily reversible if I go to a nicer designed tab / ear.
They are only there to keep bolts symmetrical,They are not a stressed part other than that.
Done up tight for the nip is what stops movements and not the tabs.
Before photo not showing for some reason, I would have loved to have seen it. One more thing to watch out for I guess.
How did the OP determine that these came off during the drive? Was it a popping sound? Or any other sound to watch out for?

Mine had done similar with the front adjusters and did not give any indication that they had. No weird handling behavior, no unusual noises, and they were actually still centered between the tabs when I discovered them, but the tabs were hammered flat on the backside of the front subframe and were working their way towards the same on the front of the subframe.
Im in the same situation as well except my rear cam tabs are busted on the driver side. spoke to Allan from Shop Formula S and he has a billet one made for our cars. the thing is, the subframe is steel and the replacement piece being billet aluminum, how will it be mounted/fused on? ill try to get an answer as soon as i find out from him. unless he misquoted and meant to say billet stainless steel which i presume is the case. i was quoted $400 for parts and labor per side.

Planned pick up this Saturday, won't see anything until then. getting quite a bit of other work done.
They finished my car today and sent me pictures. Awesome shop to work with.
They said they would wire brush it. Weld in place, and then paint it so it doesn't rust. They weld in pairs on both sides. They also fixed some of my other messed up tabs by hammering them back in place and re-enforce welding those afterwards since the metal is weakened.
