Can we solve the off-axis problem?
I don't even know if "off-axis" is the right word to use but I do know that the best listening position in my car is with my head resting on my shift knob--which makes it hard to drive.
Has anyone done anything about this in terms of realigning the mid or building kicks? Or are there some speakers out there made especially for door mounts?

Some of the better head units nowadays can do a lot of imaging tricks, which may help. For example, if you have bass in the back, you can add a slight delay to the front speakers which will make it sound like the bass is coming from the same place. On my Alpine, I have "shifted" the signal a bit so the right speaker hits my right ear the same time my left speaker hits my left ear. I don't think this directly addresses your problem, but it definitely gives you more control than the stock setup.
