The 2 step is basically a 2nd rev-limiter that lets you just keep the car floored instead of holding your foot at a certain position and then mashing it all the way after you start.
For turbo'ed cars, it also builds up some actual boost before starting so you've got plenty of power to start up with instead of waiting for the turbo to spool up.

I would like to know if this is the case (the horrid backfiring) even with a mild setup?
with any setup that has AEM EMS or some other sort of 2 step. You can set it to hard cut of soft cut, where the ignition or the fuel gets cut off at a certain rpm and it can actually blow out flames. pretty cool to watch.
