Clutch failure? Fork pivot? ACT PP/OEM Disc + PTuning Kit
That is not an OEM friction disc so I'm guessing the release bearing is aftermarket crap as well, check the markings on it and find out.
It should read Nachi and Made in Japan if it is the OEM one.
Also is that pressure plate really an ACT one? They are normally painted yellow.

But 100% thats an ACT PP. It used to be yellow on that side, it's just dirty. I would take another picture of the front but.. kinda pointless. LOL. But it's working so I can't complain.
Gonna keep the Exedy disc since it's fine and I'm a poor college student. Wish me luck ya'll.
Aside from the SOS Sport400(?) I haven't heard of any clutch setup besting the ACT/OEM combo for mild HP or even coming close in terms of longevity, chatter, pedal feel, etc.
I just recently installed an ACT/OEM setup in my car and I can assure you it's worth the wait for proper components....especially not wanting to do the job again.
As mentioned above, get the Prolite flywheel or at the very least an AP1. Even at 8ish pounds I don't have an issue driving the car in traffic etc, although my daily has had a lightweight flywheel for a couple of years now. Once the clutch has some time to break in and the initial "bitiness" is gone engagement is very smooth and predictable, with an essentially OEM pedal feel.
