Default Fuel injector popped out

so after doing a hard pull my car reaked of gas and started bogging. i pulled over shut off and popped the hood to find fuel everywhere. I start the car again and see the fuel just shooting out of the bottom of the fuel rail on the first cylinder. somehow the injector shot down into the whole more and unsealed from the top. did i miss a step on the install. I just swapped injectors, did i need anything else. this has me worried as my car could have went up in flames.

Used BOTH cushions right? (1 stock, 1 with injector kit)

And yes, that is scary!

Don't forget to remove the fuel rail mounting studs and install the 3 metal washers if you have the ID injectors.
