PhotoGenics Rolling shoot
Got a few nice shots, too bad about the lens flare though. Were you using a lens hood? Any filters on the lens? If so, what kind?
I'm with you on the hanging out the car thing, it's a bit unnerving, but not as risky as it seems. My setup is putting my 5D+35L (with wired remote) on the end of my monopod, really torquing down on the quick release clamp, and wrapping the camera strap around the monopod as a backup safety measure. I always use my lens' hoods, and in this situation use a UV filter for protection (B+W MRC, the only kind I use).
I like the monopod setup because I'm able to hold the camera a few inches off the ground. Yes, while doing ~40mph
Yes, it is still a bit crazy, do it at your own risk.

You have to remember the Photographer was hanging out of the car about 1 foot away from the street
, while we where driving 40mph plus! kinda hard to focus.......
Not to mention all the angry drivers behind us

If your camera has it (with over 1000 dollars worht of equipment I should hope it does), there is a setting that is an moving autofocus that will focus on moving objects which is perfect for this situtation.
Also try not to take pictures in the sun as everything becomes overexposed
