How often (if ever) should I replace hub assembly? They're only $200 and the hub failures seem to lead to $$$ damage.

Honda says replace the bearings every time. I've re-used them with good results but, that was just because I was too cheap to buy another set (I'd just replaced them).
The hub needs to be replaced if it's damaged. If you never spun a wheel bearing, I wouldn't replace it. It's an interference fit which means it needs to be tight so, if it's ever loose it needs replacement. If your axle or hub bolts are ever loose I imagine that would let the hubs "walk" or chatter and that would damage them. It wouldn't hurt to replace them.
You may want to consider ARP studs. I have ARP extended studs on my Civic and love them.

Those of us who change wheels frequently experience broken studs all too often. I think that the studs and/or the lug nuts just go bad after a while, with all the use and torquing.
I now use anti-seize on my studs (checking during the day to make sure they're not coming loose); I have had no studs break since I started doing that.
