Default Timing chain noise

My 03 has about 102k miles on the odometer right now and I have a problem. I keep hearing my timing chain rattle. It does it a lot now and does it when I initially hit the gas from about (2k to 3k rpm) then it goes away after that. Also it rattles when I'm idling too. About 20k miles ago I replaced the timing chain tensioner and the noise went away now it's back and more consistent then ever! I have no idea what it could be, or what I should do. Should I replace the timing chain tensioner again, or perhaps it's maybe my guides/timing chain are worn. Any help is much appreciate. Thanks.

^ replaced 2 times before 40k.....

Take it out and sandblast the worm gear part then pin it again and install. YOU MUST PIN IT DOWN IN PLACE BEFORE INSTALLING.

Or replace it for $100-$130, as a temp fix you can change your oil and you'll see it get slightly better for a while.

Oh and the reason it's worse at lower rev's is because your oil pressure is what runs it so when the oil pressure drops a little it get's worse. It'll also be worse when the motor is really hot because the oil thins out and has more trouble driving it.

Two things:

1) Check the top bell housing bolt if you've ever had it off (for a clutch replacement, etc.). Mine was completely backed out was was giving a TCT noise (card in the spokes) because engine noise was being transmitted to the firewall by the bolt head.

2) If you need a TCT, I have a perfectly good one I pulled out before finding the bolt problem. I'd be happy to sell it to you for ~ half price - say $60 shipped.
