Finding an address in Japan is weird. Even for the natives. I took the train to the nearest stop and jumped in a cab. It wasn't too far, but far enough that getting lost was a real possibility that I didn't want to face on my last day there. When you get into the cab they will ask for the phone number. He'll enter that into his GPS and take you there.
take the train to Ogikubo and get the cab from there. it's about a 5 minute ride.

Just use google maps
Especially if you are going to use the subway, it's an amazing tool.

I don't remember but it was affordable and well worth it. And I caught the bus back.
And if you're going to Japan you WILL use the subway and train system. It's awesome! Puts our crap to shame.
