One thing we usually do when the mother is in labor and delivery is imminent, we do try to stretch the vagina as much as possible to minimize tearing (and having to sew up the vagina) after delivery. I'm usually pretty gentle. With 2 fingers from each hand, insert into vagina and just try to stretch out with lots of KY jelly. One of my faculty (small, petite, young, and pretty Vietnamese OB) was absolutely brutal to her patients. She'd go in there and stretch it out like it was toffee or pizza dough. Pull, stretch, yank. Mother complain, she'd tell them to shut up, "it doesn't really hurt"

Yeah, let me lube her up and get at it with some gloves to see how she feels.

She's a biatch though. She deserves a fisting

she has horrible bedside manner, but a great obstetrician. One time, a patient needed an emergent C-section, baby was in grave danger. We're in the OR. She asks the anesthesiologist what's taking so long. He says we're waiting for the hospital pharmacy to send up some drugs stat. She said we don't have time. Told him do a local injection. She did the C-section WITHOUT general anesthesia and saved the mother and baby's life. Everybody in the OR was absolutely amazed.
