The twist in the plot is when they go to the opiate den looking place so dicaprio can test out the sedative. When he supposedly "wakes up" from the dream state he goes to the bathroom to wash his face off and check his totem. In his panic he knocks the totem spinning off the counter when the asian guy comes to check on him. He asks him if he's alright and decaprio collects himself and walks out. The scene ends and it never shows if the totem keeps spinning or stops. Everything before that is in reality but, everything from then on is his dream. The entire job the team did was a dream. When they all wake up on the airplane it's really his dream. He never wakes from the dream opium den.

the director has said the top fell at the end - if you have the blue ray version you can see that in fact the kids do age because you can pause the scene and check the information right there - solly
