That's negligent abuse and could warrant a call to the police. 

There are a lot of areas that have mandatory reporting laws. Unfortunately, a lot of people think it's not their business to make a report....And then complain about all the social problems that are connected to abuse and neglect (whether they are aware of the connection or not).
Heaven forbid that we show concern for anyone but ourselves! 
It's one thing to say "That parent doesn't dress their child properly" or "That kid's hair is horrible" because then you're right, it's none of our business.
It's another to say "That child is inhaling second-hand smoke, being blasted with overly loud music, and obviously needs some attention/assistance."

It's still not your business. Parenting is 100% opinion based and there is no right or wrong way to do it.
If they want to let their kids smoke, drink, breathe second hand smoke...that's up to them. Sure, you can think that it's wrong...but it's not your place to tell them otherwise.
And you're right, I don't show concern for anything unless it directly involves me. For example:
Katrina...yeah, it was sad...but I couldn't really care less about the people that died, lost homes, had to move etc.
The tsunami...sure, it was sad...but I couldn't really care less about the people that died, lost homes, had to move etc.
911...sure, it was sad...but ^^^
