A further search into his history shows a propensity for alchohol that is significantly higher than reported. He's given a banana bag and sedation is continued. After he is over his delerium tremens his sedation is reduced again and he is weaned from the ventilator.
On Post operative day 6 he is extubated and I visit him and we have a short conversation. He is tired, but completely coherent. He thanks me. He is happy to be alive.

The cause is unknown. Many times it's attributed to hypertension and shearing forces into the multilayered wall of the Aorta. Some think it's a defect in the formation of the Aortic wall in certain individuals that makes them essentially ticking time bombs.
