CDMA phones need BitPim, I believe. most of those phone have proprietary systems for ringtones and you need to gain access to the phones system to edit.
blackberrys and others like them can use MP3 files easily so not much problem there

i have a mac, im sure there is similar software for windows for free but this is what i do.
1-Pick out the song in itunes or any other software that plays music (even have done it from myspace music/ect)
2-queue up the part in the song i want to create my ring tone
3-open a free program called "audio hijack pro" - http://www.rogueamoeba.com/audiohijackpro/
4-start hijacking my audio (i usually create ringtones that are ~35 seconds in length)

They were a huge pain to make for my early RAZR. I used Audacity to convert various sources to the right format (MP3's IIRC) and bit-rate. I had to use some odd freeware to access the phone's directories and transfer my tunes. Even then not every created tune played correctly as a ringtone.
My main ring is now the Green Hornet theme song from the Kill Bill soundtrack CD. Found some other classic TV themes on a big collection site, forget the name.

This is the best, easiest and free way to do it. Takes 30 seconds to do a ringtone and you dont need anything but itunes.
Follow these steps and you can make anything in your itunes library a ringtone.
This is for Windows, most steps are the same in MAC, i just dont know the menu options MAC has to change them
-Open iTunes
-Right click the song you want to be a ringtone
-Click "Get Info"
-Goto the options tab
-Check mark start time and stop time and input the corresponding time you want (You need to have figured out what time frame of the song you want to be your ringtone)
-Click OK
-Right click the song in iTunes again and click "Convert selection to AAC" (this will create a duplicate of the song in M4A format of the part you chose
-Right click the duplicate you made and click "Show in windows explorer"
change the extension of the file to MP3 and save it in a ringtones folder or somewhere easy for you to find it
-Load it on your phone
This is also a good trick if you have a iPhone. you just change the extension to M4R instead of MP3 and sync to your iphone
