Waiters/Waitresses: 0% to 35%, 15% average. 0% for having to go to the cash register just to get my bill- waiter never came back after serving water. 35% if they're damn good or she's pretty good and hot (helps if they look like a student). IMO, service overall seems to have gotten worse in the past few years and they seem to expect 20% or more- BS in my opinion.
Barber: 15-25%
Bartender: $1 per drink or $0.50 per glass of water (if I'm ordering water, I order 2 at a time- one for me and one for my girl)
Pizza Delivery: $2-4 (I used to deliver pizzas occasionally in well-off neighborhoods, and there were times when I'd get no tip at all- for the most part, the tips barely covered the gas for my car. There'd be days I'd make about $7 in tips- most people tipped by rounding up to the next dollar.)

If I'm out with a girl for dinner then the lucky waitress/waiters will get hella $$$$. If I'm out with my friends then its whatever pocket change I got in my pocket and maybe a dollar or two.
Haircut is the most common and for a $14 haircut I do 2 bucks. I'm a broke graduate school student, If I was rich then it would be diff. Every dollar to me counts.

I work in a resturaunt, so i am sure if I get good service to tip, but I have learned to tip directly to whoever waited on me, at times 2 people (like at a mexican resturaunt, the dude that gets your chips and your waitress). This is because i have worked in a couple resturaunts with lets call, fvcked up tip policys to put it simply

all of my friends say i tip too much, but i don't care(most of the time 25%tips). being a waiter/waitress is hard(in most cases) and i know what they're going through. i believe in the "what goes around comes around" idea, and it's been working out great.
but i will give a 0% if they are very rude or make too many mistakes that could have easily been avoided.

There have been times I didn't leave a tip at all if the service was so bad. I went to a big italian restaraunt where I never got a refill on my drink the entire time I was there. Even after the waitress brought the food, we didn't see her again until we got the check. I wrote in big, fat letters on the bill "REFILL MY DRINK NEXT TIME!"
Usually I'll tip between 15 and 30% depending on who I'm with and the service.
But tipping is always optional...
Has anyone done this before:
Put down a stack of ones and tell the server that this is your tip and each time your drink isn't filled, you take away their tip and so forth? That's kind of being a jerk, but I bet it would be fun as heck to do.

waiters - i just double the tax (which turns out to be 16.50 %) if the service is good, if it's rough then they get change in my pocket.
pizza guy - 1-2 bucks if the weather's good, 5 bucks if it's raining HARD
