are they asking you to put in the name of the mother or father thats not there for the child while they renew the kid's passport? Question 3 <<.
confused about question 4a and 4b. pick one which one if the mother of teh child wasn't there? 4b? and write an explaination of why she isn't there?

Is this for your child? I believe it goes like this:
For Q3 put down the relationship to the child(ie. Father, mother or legal guardian).
Fill out 4a if you aren't applying for a passport.
Fill out 4b if you are applying for a passport and another guardian who is not applying for a passport cannot sign 4a.
If you want to be sure, your local post office should be able to help.
