Stupid airlines -
In all fairness, they would need to start charging based on body mass index or something, as you'd have the guy who is 6'8" and 290lbs being charged pretty heavily just because he's taller (and therefore wider).
I am not huge, but I guess I'm somewhat substantial in size at 6' tall, 230lbs and my shoulder width causes me to have to a) spill over slightly on to neighboring seats b) spill into the isle and take elbow hits from the food service carts c) sit twisted in the seat so I am less wide.
I dunno what the solution is, but maybe it's time to come up with some other forms of high-speed, long distance travel. Find a way for us to sleep on the trip and pack us in like sardines.

The solution is there, so to speak. And that's to pay more for the privilege of flying. Whether in 1st/business class, or just accept that it should cost $800 to fly across the country. The sardine aspect of flying is a direct response to people demanding cheap tickets and/or their unwillingness to pay more to fly farther. A NY to LA flight uses $150 of fuel per person in a 737, but people still complain when their round trip ticket is $400. Pay more, or accept that you're going to be sardined.
To a degree, I agree. I've learned to travel lightly and it always bugs the shit out of me when someone brings 2 giant bags (each weighing 49.9 lbs), a carryon and a "personal bag" that's is as big as me... So in a sense I don't mind the 2nd bag fee, even though it creates a perverse incentive for people to try and cram more into the already overflowing overhead bins.
I've never liked the airline pricing system. It's never made sense to me to constantly change the price of something that has a fairly fixed cost, and of all the flying I've done the last 10 years, I suspect the airlines lost money on 90% of my tickets. That's a lousy way to do business.

All carry ons should be strictly limited to a single bag of fixed size, no duty frees etc., except for those paying more for Business & First where the should be allowed one extra bag for business and two extra for First. The same should apply for checked baggage.
BTW the economics are such that, once First is full (20 passengers approx) the rest is profit on international long haul flights. The First and Business pay for the rest, hence the cheap seats in cattle class
