In Saving Private Ryan, I believe the flub is in the last quarter of the movie. When some soldier waits for a tank to pass him while he hides in a ditch. When the tank passes by him, he gets up and sticks this grenade cover with sticky shit onto the tank. While he's doing this, you see a heavy grease stain right above where the grenade is, where he stuck it before during rehearsal.
In The Matrix during the lobby shootout Neo is shooting two Beretta sub machine guns. They are 9mm caliber and the shells flying out of it are 9mm. When they switch to a floor shot the shells hitting the ground are .223 shells from the M-16's. I'm really into guns so things like that and the constant never needing to reload really bother me. I love movies when they can actually get little details right.

For those that remember the 80's film "The Last Dragon". . .
During an early scene of the film when the younger brother shouts to his brother on the roof he says "Yo Reloy!. . . come inside. . ."
The only problem. . . The main characters name was Leroy. . . Not Reloy. . .
Something tells me the budget forced them not to redo the scenen. . .
I Still crack up everytime I see the scene. . .
