(power cable length) should I put the fuse/fuse holder at?

Closer the better, I think.

 

I have an inline fues holder that is about 5 to 6 inches from the battery. You will want it pretty close to the batter in the event that the power cable insulation or some other short occurs on the cable.

The installers rule of thumb is within 18 inches.

 

Newer rules of thumb are less than 12. As said above the closer the better! Just in case there is a problem in an accident you dont' have a loose wire hanging around.

 

How about the ground wire, how long is the maximum length?

Rockford Fosgate has an excellent Fuse holder that attaches to the battery. Thats what I use. Its about $17 or so.
