A too cold knob will be very uncomfortable. A too hot knob can send you to the hospital.

Hot is worse than cold. If its that cold out you are probably already wearing gloves. If its that hot out, you only have driving gloves or a knob cozy if you prepared ahead of time.

I have the OEM Ti knob on my daily driver (I have an Arvou knob on my AP1)

I haven't noticed it being too obscenely hot.

But I've also had metal shift knobs for the better part of the last decade, so at this point I just expect a hot shift knob.

I've used the oem Ti knob for years in extreme hold and cold and it's never been uncomfortable. I've had the oem Al knob in other cars and that was not pleasant.

Titanium has almost 10 times lower thermal conductivity compared to aluminium, meaning it will feel much more pleasant touching a Ti knob when hot or cold.

My knob rarely sees the outside in the winter. I try to keep it inside when it is that cold

OEM Titanium knob here. I didn't feel much difference compared to the aluminum version.

I do feel some warmth that radiates up from the transmission when I park the car for a hour or two, and I climb back into it.

I'm surprised it took until post 15 for it.

This is FL "winter" so it's probably pretty mild compared to what you're used to. Usually 30 overnight and 40 in the morning when I would be in the car. I would turn the heat on and put the top down.
