I just experienced the problem for the first time today.
I guess it was an honest mistake on my part though, I usually tighten to one click (Been doing it for half a year and this is the first time I've seen this problem).

The service manager from the dealer called back and said that his research
said that this is a known problem for S2000s. The threading is defective in
the filler pipe so the gas cap does not stay in place always. He has ordered
me a new filler pipe and gas cap and HondaCare will pay for it.
I only wish Honda can be a little up-front about these things and make these a TSB so every owner does not have to go through my experience. So, if you had these probs, hit up your dealer.

It took me 4 visits before I could finally convince them it wasn't just me being a retard, but I finally got mine replaced. The trick is to have them "tighten" it until they're 100% certain it's engaged, then go and loosen it with just your pinky.
