Here's mine, happened sometime last year. A lady pulled out in front of me in a van, my car went under the van slightly, there were two lanes in my direction, she pulled out to my right, me being in the left lane, then she decided to change lanes 10 feet in front of me, I'm 45-50mph and she's 10mph.

Nope, it was within feet of the intersection (I had green light), we were driving into the sun so vision played a factor but there was still no way I could have stopped.
