I think I put my left hand on the body of the car, near the top, but I honestly don't remember for sure.
This is a stupid thread but I will chime in anyway. I put my left hand on the black knob used for the door crash safety mechanism. It is black and about 1 inch in diameter, about 2 inches above the sill, and makes a great handle to push off to make a graceful exit from the car. I still have not found out a way to make a graceful entrance though.

When exiting I generally put my left palm in that exact spot shown. Putting weight on my left hand and my right foot (with the right hand on the wheel), I kinda lift myself over the bolster, onto my left foot.

I taught my g/f this method and she now looks a lot more graceful entering the car.

Left hand goes on the door sill. Right hand goes on the center console. Lift body UP and over the bolsters, step out w/left leg, transfer weight to left hand on sill, step out of car w/right leg.

Another vote for push myself up off the doorsil. Don't want to crush those seat bolsters. It is a PITA if have have a bunch of shit and I'm trying to get out of the car- keys, iPod, sunglasses, bag, etc. Only have two hands...
