Default Clutch trouble

alright so here's my problem fellas..02 AP1, about 6-8 months ago replaced my clutch disc and pressure plate, resurfaced the flywheel. went with ACT heavy duty pressure plate, ACT street organic disc, and kept my prolite flywheel, and had a machinist friend of my mechanics resurface it. i replaced the TOB and Pilot and all was well until about a month ago. clutch pedal went all mushy, i replaced slave cylinder, it drove great for about 12 hours until i started driving "spiritedly". the clutch started slipping, progressively getting worse. i was going 22 MPH and the revs would go all the way to redline if i didnt use anything but the softest pressure on the gas...so my question is could it just be the master cylinder, or is it the TOB, or worse, all of it? Any help is super appreciated!

Your clutch is shot.

Or, the master pushrod was way over adjusted and not allowing the master cylinder to return.
