Competition stage 2 clutch failure
So the bearing snapped in half. The disc has delaminated. Had it on for less than 3 months about 500 miles. Car is NA. Flywheel has been resurfaced. Attempting to claim on warranty which is for 90 days. I purchased it just over that unfortunately. Will let you know how it goes.
Common failure. OEM or OEM/ACT PP is the go to recommendation for clutches unless you need more extreme clamping force.

If they warranty this, you should expect the same failure again. It's going to keep breaking.
You really need a Honda genuine disc (or a disc with enclosed springs), and Honda genuine bearings.
You also need a new release bearing guide sleeve and urea grease from Honda.
