Your Honda dealer is the best place to order all except the shim. They make you buy the whole kit of over 2 dozen shims, of which you use only one. Cost is like $200.00 US.
You will need what I showed in that first pic in that thread I linked:
-pinion lock nut
-crush sleeve (get a second one just in case the installer ruins the first one)
-inner pinion race and bearing

To do a "proper" job, you should also get the outer pinion race and bearing, both output shaft seals and pinion shaft seal.

The shim must be custom made at the time of calibration. Your installer needs to have access to a machine parts shop that can make this shim with short notice. Otherwise, you pay Honda $200.00 for the whole kit of about 2 dozen shims.
Rick's gear set no longer comes with shims because it is too hard to know exactly what thickness you will finally need so there is no point in including a shim on an educated guess.

Not necessarily. Your installer needs to have "Hondabond" or its equivalent. You may also need the output shaft seals if the stock ones are not well looked after during the installation process (ie. damaged). Of course, you will need the correct diff fluid.

For clarification of Stratocasters schematic:
#10 = crush sleeve
#23 = pinion shaft lock nut
The rest is self explanatory.
