Here's some possibilities:

Damaged synchro
old/contaminated trans fluid
contaminated clutch fluid/sticking slave cylinder
rusted splines on the mainshaft, not leting the clutch release.

Change the fluids 1st, then see if it does it again.

Make sure it is of the Friction Modified variety.

No. It can be hard to find, especially since there are at least three different GM procucts called "synchromesh manual transmission lubricant". And each one can have a GM part number or an AC-Delco part number.

The dealer I bought some from didn't know it existed or that he had any, but then he went back in his shelves and found three quarts. (Also, I was charged about $11 per quart, so the price from Hardtopguy is pretty good.)

It takes more than one. I ended up using all three bottles, but there was some spillage and some fluid left in the pump. Also, the pump I was using wasn't able to get the last few ounces out of each bottle. I drained the dregs into the other bottles. Ended up with a full bottle left.

I would buy three just to be safe.
