Default HELP! Blown Slave Cylinder

2001 S2000, 78k miles. Replaced clutch, bearing, pressure place with Exedy OEM replacement parts. The car started blowing out the slave cylinder boots once the engine had been shifted several times above 6,000 rpm.. Replaced the slave 2 times. Removed the transmission. Installed HONDA clutch disk, pressure plate, flywheel, bearings, tube, fork & cradle, slave cylinder. Drove about 30 miles and the slave blew out again. It acts like the slave rod is over-extending. I am at a loss. Anyone had a similar issue with a solution? Thanks!

The acordian black boot is blowing out, leaking fluid, but the piston is not becoming unseated off the fork, or the slave...is this correct?
