The fatigue is from the internal stress of the rotor as grows and shrinks from heating and cooling. The actual mechanical load from braking is negligible in comparison to the dimensions and strength of the material involved.

It is a factor - and a very important one. You have two factors at play. The swept area (the total area of the rotor that the pad can contact. And the actual contact area between the pad and rotor. When you drill or slot you reduce BOTH. When you reduce EITHER, you reduce braking capability.
