Member ID Card and a decal?

I've been a member for ~4 years now and have never received a decal. Am I missing something?

 

I got mine when I re-subbed last August, but not when I first bought membership. Did you fill out a Google Doc form when you last paid for membership?

 

During your renewal, there should have been a prompt to fill out the address form IB uses to send membership items. You could have missed it, which is not uncommon. Or are you set up to auto-renew?

 

Totally didn't see that at all. Not set to auto-renew either. My subscription actually expired and manually renewed it today. Any way to retroactively go back and fill it out?

I must have been missing that form for awhile now. The last card I got was in 2006.

So is there a way to get a card and decal?

And I do not remember getting an entry form for the "Members-Only Giveaways".

And, I'd also like to know how to get that ID card and decal, as well.

I never seen the form until a few weeks after renewal in an email, I filled out the form back in December but never received anything. Can anything be done with this as I purchased a Gold membership with the bells and whistles?

Just paid for my gold membership moments ago, it's been overdue for nearly 4 years.
Was told cards are no longer available, unfortunately

But, still looking forward to the other stuff .
