Default SQL errors and duplicate posts

May be totally unrelated issues but wanted to report two things:

1) Tonight I got an SQL error when sending a PM (error message below). I got an SQL error on something else (page load, or maybe it was a post), but I can't recall the specifics of it. Figured it was a one-off so I didn't pay it much heed. But now with this, I figured it might be a trend, so best to report it as a possible problem.

SQL Error
An error occured with the SQL server:
mySQL query error: SELECT * FROM ibf_cache_store WHERE cs_key IN ( 'notifications' )

This is not a problem with IP.Board but rather with your SQL server. Please contact your host and copy the message shown above.

2) I have noticed a significant increase in duplicate posts lately. In the last week or so, there has been an up-tick in duplicate posts in various sub-forums. I notice them in S2000 Talk and Vintage mostly, but that's because I have to clean them up there. They may be appearing more frequently in other sub-forums as well, but I haven't tracked that. People certainly sometimes goof and accidentally post threads or posts twice, but this seems like more than an individual's goof. Seems indicative of a site or connectivity issue.

Admins can see some examples (dups are unapproved) in this thread:

Also, anyone can readily see dup posts in Status Updates if you read through the replies.

Dup posts. I use the forums mobile app. Often when I try to submit post I get stuck in an endless loop with a spinning progress counter, just spinning away. If I touch the screen anywhere, it stops, and I am back in my post like I was still typing it.

Sometimes when this happens it did actually post, sometimes not. So if I back out back to viewing the thread, if my post is not there, I have to type it all over again. If instead of backing out I try to submit again, it works, but then sometimes that means there will be a DUP post.

Maybe this is happening to others as well, and this is where all the dups are coming from

I always go back and edit the DUP and say something derogatory about the mobile app.

...and it just happened trying to post previous comment^. Endless loop, I had to press Send a second time, no way to know if it would DUP post.

It didn't. This time.
