Default Crazy Flywheel

Well as me and my buddy were doing the clutch job for his 01 teg GSR, we found out that the flywheel bolts BACKED OUT on their own. As we dropped the tranny, took off the pp, and clutch disk we find that one flywheel bolt had completely backed out onto the clutch disk, tearing up the springs and minor damage to the steel backing. The rest of the 7 bolts, 6 of which I was able to HAND loosen them, only 1 bolt was still in torque spec. In my life, I've never seen such a thing happen, any inputs guys? I wish I had a camera to take pics with, but I didn't .
