milk tea is nai cha. nai cha doesn't signify the usage of the black balls (vs. say.. blue balls?).
boba, above and beyond being extremely difficult to write in Chinese, means tits in taiwanese slang. so you're literally drinking tittified milk tea. gwailo couldn't really understand "boba" when introduced the tapioca, and started saying 'bubble'. chinese-american biz men, in order to further gain traction in the US market, started selling boba as 'bubble' in certain regions.
in taiwan/hong kong, if you ask for bubble ***, vendors would be like: finally, a new restaurant, franchised by the most popular boba shop in Hong Kong, just opened up in Chinatown underneath Phoenix:
