What type of router do you have?

http://support.xbox.com/support/en/us/xbox...connection.aspx

When I got FIOS, I ended up making sure that my rules were setup to my device name vs. IP address (DHCP). That way it keeps the port(s) opened for that device.
It's a Netgear, not on the Xbox list.

All the right ports are opened, I tried to put the Xbox outside of the DMZ, cannot for some reason.

Hmmmm....

Did you always have this issue? Also, when you run the test, what does the NAT level say?

I want to say that I had enabled/disabled "UPnP" on my Netgear and it had fixed it when I was having issues with it.

Or it could have just been trouble with the COD servers last night

Mark (speedbump) hooked my up with the router, I had a wired 4 port before.

I will run the tests again and let you know the result.

I had issues with another game, racing in tournaments in PGR4 right after I added the router.

There is a NAT issue, regardless of the servers last night.

All the right ports are opened to the correct host? Your Xbox have a Static IP address? That should work...
