Got a Tech question for the Mid-A Guys!

So guy's I did a leakdown and it is deffinitely a exhaust valve, on cylinder 4.
I'm going to send it to alaniz but the question is why did it happen?
Car was NEVER over revved. I don't want to put a new head on and have the same thing happen. Could it be a burnt valve? Oiling issue?
