Attn: serious golfers
Here are my guesses according to experience.
1) Check your grip. It may be too weak, which means your right hand is too far on top of the grip.
2) You may be clearing your legs and hips too quickly. This causes the face to be open at impact.
3). Swing path bay be too upright and out to in. Attempt to swing 'down the line' or and an 'in to out' path.
This comes from a guy who 'used' to play on the Golden State Tour. These tournaments made me noticed how shitty of a golf I really was as a 2 handicapper.

On my last benge of errant tee shots (and fairway woods) I had a problem with keeping the same grip pressure throughout the swing. Too much right hand = left, too much left hand = right. If your ball is starting out where it is supposed to be going then fading/slicing then that could be the problem.
