Sorry, BS flag on this one. If you are working until failure, you don't have the time to check your surroundings and check for people's toes. If you can no longer hold the weight, it drops immediately. If you DO look around and scan your surroundings, that means you can easily place the weight on the floor with no problems.
Put earbuds in. Problem solved.
The gym I work out at has a lot of pros/big guys (Powerhouse downtown Tampa). It can be as intimidating as it motivating... but when they're squatting 600LBS and benching upwards of 400.... my "grunting is annoying" complaints are hardly valid...

at a certain level of training the exhale is almost necessary. Its a big part of alot of sports i.e. tennis, power lifting, karate etc.
my friends and I had to relocate to a "meathead" 4 walls and mirrors style gym because one of us had a "grunting" problem.
I can see it being a problem at normal ymca and chick style places, but if you have barbells that are over 100 pounds you are asking for meatheads to be there to begin with. If you lower the amount of weight there you can eliminate the meatheads before they even sign the contract. trust me.

I am more impressed by the mirrors in the first video....Did they withstand the barbell being thrown at them and not break or was it just my eyes not seeing the breakage while the camera panned to follow the asshat?
