For me,

On top of the fitted sheet, Under the..

Bed Sheet,
Second Bed Sheet
Fleece Bed Sheet
Duvet
Comforter

I like having a lot of weight on me while I sleep, and when alone, I bank pillows up all around me, why I have a king size bed I don't know?? but I like the cocoon feeling. I think a coffin would be the perfect bed for me
I'm the same way. I'd be the first in line for a lead comforter if they made one

As for the hotels, I masturbate on the sheets and sleep on the floor

Really though, I don't care to touch the comforter or that camel skin crap they always use. I just make sure I'm inbetween the sheets b/c they atleast wash those all the time.

Interesting. At home I often just sleep with the comforter. I have the sheet that goes below the comforter but I never really use it... Hummmm...

Over the fitted sheet.

Under the flat sheet and a blanket. I should look for another one of my nice blankets so I can keep a window open through the 0 degree nights.

And when it is cold, like this time of year, a quilt is added.
