I dive a alot and having swum with large stingrays multiple times I can say this has to be one very freak accident.

People usually get stung by smaller sting rays when they step on them. Since he died by the stinger going thru one side of his chest and into his heart, rather than from venom, that means he has to have been swimming with the big ones.

If he was swimming with the larger rays, it was probably in a feeding situation. They are very aggressive and try to steal the food from the other rays so it is not unusual to have rays all over you, but I have never heard of a person being "hit" by the barb. We aren't a predator to them.
