Mannn sorry to hear that!!!
I gotta go check all of my bolts now... I haven't done a full extensive check of all bolts yet...
We REALLYY need to find a good solution to all of these bolts loosening...
You should just replace the nuts that came with the kit to stover nuts and nord-loc washers,I use this combo on my turbo kit and recommend them to anyone that has a problem with the nuts coming loose all the time.

I also forgot to mention that one of the bolts that holds the center section of the turbo to the exhaut housing also backed out.

As mentioned previously i've used nord lock washers and safety wire, locktite, etc
but I again i'm positive about he whole thing and i'm blaming not having a brace on the down pipe to the engine.
So I got this done last week. However, come home and the bolts for the brace were gone too
So nothing stays tight on this damn car. So I double nutted the brace and it should be ok since not a lot of hear raidates onto the u-clamp.But yeah my bolts used to disappear from the housing never to be found again. And to get to the front bolt towards the engin is not happening and the whole assembly has to come off and it took me hours. And the compression housing is PITA if you are running a 35R.

There is little to no room working on that side of the car. I could only imagine how fun it is when you have coolant lines running to your turbo.
For those who don't know the compressor housing on the Turbo has to be removed in order to remove the turbo. I will give you a brief breakdown w/ no pics but you will get the idea

Here is a quick solution you guys can try that I have done before and it works. If you happen to be using a nut, place it on it's side on the anvil of a vise or something equivalent and smack it with a hammer to just barely deform it. this will create just enough friction to keep it from backing out.
If it's a bolt, place the threads on the anvil and tap with a hammer to dick the threads a little. Be careful if your bolt is going into aluminum though, you don't want the threads dicked up too bad or you will gall the threads up; just enough to make the bolt have more friction.
