Mine is pretty dumb. I was driving my mom's Toyota Highlander on the 91 freeway a few years back. I decided to downshift to pass some cars and while doing so I ended up slamming the brake pedal and throwing the auto lever into neutral... millseconds after realizing my mistake I went WOT bouncing off the rev limiter.

Now whenever I drive an automatic I actually bring my left leg up and far enough to the left just so I dont make the same mistake again. In my wife's last car, you can actually see the wear marks on the driver doors fabric from my knee.

About a month ago, it had just started raining, and my coworker asked me to do a quick wheelspin/burnout in the parking lot. There were a few people watching, so I figured hey, why not. Revved up just a little to ~6Krpm and slipped the clutch sorta quick. VRROOOOM VRRROOOM! Wait, what's that smell? Nice, burned the clutch. Smelled it pretty much all the way home. Way to go shamoo.

I couldn't find the gas release the first time I went to get gas and had to call a friend who had owned one,
I also accidentally hit the mute button and I got pissed that my music wasn't playing, so I kept trying to turn up the music to see if I was just not hearing it and then when I finally found the mute button and pushed it my music was blaring and it scared the holy BLEEP outta me. It also took me about 2 mths to find the "secret" compartment.

I once tried to raise the roof with the tonneau cover still in place. Doesn't work.

I've lowered the top a couple of times with the tall (aftermarket) windscreen laid down. Fortunately, I've always noticed the sound before breaking the plexiglass.

Not in my S but many many years ago I backed my car out of the garage with the passenger door open. Not good. Went shopping for another door at the junk yard.

I left the owner's manual on the roof and folded the top down with it stuck in there.... needless to say I was at least pretty glad that my rear window didn't break on my 2-day-old S2000.

after taking my brand new car from the dealership straight to pick up my girlfriend at her house, I made a U in the parking lot and needed to reverse out because I didnt make it in one go. we sat there horribly "parked" for a good 5-10 minutes while I called the dealership how to reverse =]
and the top was down

I never tried reversing in a 6 speed!
